WAIVER In consideration of being allowed to participate in the personal boxing training activities and programs of P4P Boxing & Fitness, LLC and to the use of it’s facilities, equipment, and services in addition to the payment of any fee or charge, I do hereby forever waive, release, and discharge P4P Boxing & Fitness, LLC and their officers, agents, employees, representatives, executors, and all others acting on their behalf from any and all claims or liabilities for injuries or damages to my person and/or property, including those caused by the negligent act or omission of any of those mentioned or others acting on their behalf, arising out of or connected to the use of any equipment at various sites, including home, provided by and/or recommended by P4P Boxing & Fitness, LLC. Buyer agrees to use the facilities and equipment of P4P Boxing & Fitness in a safe and responsible manner and to request necessary instruction on the proper use of equipment or form when needed. Buyer also agrees to use necessary protection (i.e hand wraps, headgear and groin protector) during workouts. Buyer also agrees to properly warm up prior to beginning any exercise program. Buyer also agrees to consult with PHYSICIAN FIRST if the buyer has any existing medical conditions. Buyer consent to P4P Boxing & Fitness Studio plus its affiliates and agents, to use my image (both photo and video) and likeness and/or any interview statements from me in its publications (including but not limited to Social Media), advertising or other media activities (including the Internet). MONTHLY PRICES, PAYMENTS AND BALANCES DUE It is understood that the first month’s dues will be collected upon joining. Monthly payments will be collected through an Electronic Funds Transfer (EFT) system using a major credit card or checking account. Any accounts declared past due as a result of insufficient funds or any other issues related to non-payment will be subject to a $35.00 service charge. Failure to correct this within seven days of written notice from P4P Boxing & Fitness, LLC will result in suspension of membership and will be subject to a $50.00 reactivation fee. Buyer acknowledges and accepts a one time Annual fee in the amount of $19.99. The fee you pay is dependent on your join/sign up date (commonly mid-way through your membership) is your annual maintenance fee for studio improvements such as new equipment, making renovations and/or other much-needed improvements or repairs. Annual fees are billed to a member once per year. Buyer acknowledges The New Jersey (NJ) state sales tax rate is 6.625%. Buyer understands all that his/her monthly membership package includes and that a P4P representative thoroughly explained, and buyer agrees that any additional service(s) such as Personal Training (PT) sessions come at an additional expense above the monthly membership price listed in section above. P4P Boxing & Fitness, LLC reserves the right to put members into collections for non receivement of payment at any time. Buyer agrees that this membership shall be valid and kept in good standing for no less than 3 months (or 90 days) which shall include no less than 3 billing cycles. Buyer Acknowledges this agreement shall be deemed worth no less the monthly membership price listed above multiplied by 3. Guest Privileges: The guest must be accompanied by you. The guest must sign a medical and injury release form each time they use the club facilities. The guest must follow all rules and regulations. The guest must prove that he/she is at least 18 years of age or older or is at least 14 years of age and you are their parent or legal guardian. You may bring a different guest each time or the same one-it's up to you. Freeze Policy: Only EFT memberships may be frozen for a fee of $5.00 per month while on freeze. Must be frozen before the 27th of each month.
